Referral

Please contact Help Me Grow if you have questions or concerns about your child's development. Referrals may be made by parents, child care providers, preschool teachers, physicians or by other professionals (with parent permission).

Make a referral for children ages birth to three years of age by:

calling Help Me Grow (952-368-0160)
emailing Help Me Grow at helpmegrow@cseced.org or
faxing a Referral form) to Help Me Grow (952-368-0168) (this option requires parent signature).

Please provide us with the following information:

your child's name and date of birth
your name, address and phone number
best time to reach you
your home school district (if known) and
what your concerns or questions are.

We will call you to follow up.

Make a referral for children ages 3-5 years of age by calling your local school district:

Greater Metro Area
If you live in a community outside of Carver County and would like to make a referral for Early Intervention Services:

You may refer to the Metro Early Childhood Intervention Card

and call the school district or central intake number for your community.
OR
You may use the Referral Form

and fax it to the number listed by your city of residence.
